On October 19, 2023, Continuum Health detected suspicious activity on its computer network, indicating a data breach. Based on a subsequent forensic investigation, Continuum Health determined that cybercriminals infiltrated its inadequately secured computer systems and thereby gained access to its sensitive files during the period October 18, 2023 through October 19, 2023. The investigation further determined that, through this infiltration, cybercriminals accessed and potentially acquired files containing the sensitive personal information of 377,119 individuals. - Medical information
- Health insurance information
- Contact information
